Your First Visit

During your first visit you will also be asked to complete a comprehensive health history form. If the appointment is for your child, you will be required to fill out a form for him or her, as well.

You can download the Comprehensive Health History forms here so that you can complete them at home. You can bring the forms with you to our office or you can fax them to us prior to your appointment at 203.288.1851
